2. Attach Attestation form found in Parent Resource handbook 2025-2026 pg. 17-19
3. Attach copy of Tutors credentials such as high school or higher education diploma
4. Attach invoice that contains Name of Student, amount or charges for tutoring, interval of invoice such as monthly or quarterly
